CCC is here to help fulfill your Winter Wishes!

Eligible families who are undergoing active treatment are referred from Blank Children’s Hospital and University of Iowa Stead Family Children’s Hospital. If you were not referred directly from one of these hospitals, you are likely not eligible for this program for 2023. Please email the Programs Team with questions.

If you were referred by a hospital, please complete this form to create your child(ren)’s wish list(s) and select a gift card or cards of your choice. Please be sure to add specifications on requests (colors, sizes, etc.). 

Each child age 18 or younger in your immediate household may receive up to $150 value in gifts. Immediate siblings older than 18 residing in your household are eligible for a $50 gift card.
